Waiver and Release of liability

All adults traveling with Pursue Missions are required to complete the Adult Travel Liability Waiver, which can be filled out online using the link above before their trip.

For travelers under the age of 18, two additional forms are required — a Minor Consent Form and a Medical Release Form. These forms must be picked up from the Pursue Missions office, notarized, and returned to our office before the participant is permitted to travel.
